Microneedling vs. Derma Rolling: Why Professional Treatment Wins
A $20 derma roller and a professional microneedling session both involve tiny needles, so it’s easy to assume they’re the same thing. They aren’t. The difference comes down to depth control, safety, and how consistently the treatment can work, and those differences matter for your results.

What is a derma roller?
A derma roller is a handheld drum covered in small needles that you roll across your skin. Rollers sold for home use usually have short needles, and the depth you actually reach depends on your pressure, angle, and technique. The FDA notes that products with short, blunt needles that only claim to exfoliate or improve how skin looks are generally not regulated as medical devices, and that it has not authorized any microneedling medical device for over-the-counter sale.
What is professional microneedling?
Professional microneedling uses a medical device to create controlled micro-channels at a chosen depth. At Dixi’s MediSpa we use the Candela Exceed™, which lets your provider adjust depth and speed to your skin type and the area being treated. Learn why in Exceed vs. traditional microneedling. For a neutral overview of the professional procedure, see Cleveland Clinic’s patient guide.
Side-by-side comparison
Derma roller (at home) | Professional microneedling | |
Depth control | Limited, depends on pressure | Adjustable by device and area |
Needle motion | Rolls, can drag and tear skin | Stamping/gliding with controlled entry (device dependent) |
Sterility | You maintain it | Single-use sterile tips |
Provider assessment | None | Skin screening and plan |
Add-ons | Over-the-counter serums | Exosomes, PDRN, medical-grade topicals |
Results | Mild, inconsistent | More predictable collagen response |
Risk | Infection, uneven results, irritation | Lower when performed by trained staff |
Cost | Low upfront | $350 and up per session |
Why professional treatment wins
1. Depth matters. Collagen induction happens at specific depths. At-home needles are too short to reach it for most concerns.
2. Technique matters. Rolling can drag across the skin. A medical device is designed to enter and exit cleanly.
3. Sterility matters. Used rollers can harbor bacteria if not cleaned and replaced regularly.
4. Products matter. Applying the wrong serum to freshly needled skin can cause irritation. Professional treatments use products designed for that moment.
5. A plan matters. Texture, scars, and pigment each need a different approach.
What about derma rolling for hair growth?
People often search for derma rollers for hair loss. The evidence is limited, and hair loss has many causes, so a diagnosis matters first. We don’t offer hair loss microneedling at Dixi’s MediSpa. If you’re noticing thinning, a physician or dermatologist is the right first stop.
When might a derma roller be fine?
If you’re set on trying one at home: use only on healthy, clear skin, clean it thoroughly, replace it often, never share it, and avoid it over active acne, rosacea, or sunburn. The FDA also lists infection and other risks of at-home devices. But for scars, deeper lines, or pigmentation, professional treatment is the route that works.
What professional microneedling costs
Option | Price |
Classic microneedling | $350 |
With exosomes | $525 |
With PDRN | $595 |
The recommended protocol is 3 sessions, 4 to 6 weeks apart. See the full microneedling cost and results guide.
Frequently asked questions
Is a derma roller the same as microneedling? No. Both use needles, but professional microneedling controls depth, speed, and sterility.
Can a derma roller treat acne scars? It’s unlikely to reach the depth needed. Professional microneedling is a common option for acne scars. The AAD explains how microneedling can fade scars.
How often can I use a derma roller? Per the manufacturer’s guidance, and less often than you think. Over-rolling can irritate skin.
What’s the downtime after professional microneedling? A slight pink glow, similar to a mild sunburn, usually fades by the next day. Skip makeup for 48 hours.
